What are my patients' choices?
Atrium Health CareConnect offers many benefits to patients but their participation is voluntary — patients get to choose whether they want their information included. We encourage you to first talk to your patients about this decision, using provided talking points and have patients review our patient FAQs about Atrium Health CareConnect.

Opting-Out of Atrium Health CareConnect

Patients who do not want their medical information to be accessible to authorized health care providers through Atrium Health CareConnect may choose not to participate or "opt-out." If a patient chooses to opt-out, healthcare providers will not be able to look for their records in Atrium Health CareConnect. Choosing not to participate means emergency room (ER) doctors will not be able to get information that could help them give patients better care or save their life in an emergency. Also, some providers may decide not to see patients that do not participate in Atrium Health CareConnect because they will not have access to medical information that would help them give patients the best care possible.

How to Opt-Out:

If a patient does not want to participate in Atrium Health CareConnect, they must complete an Opt-Out Request Form. By completing the form, the patient is only opting out of the Atrium Health Connect. Patient records will still be available to you and other health care providers from sources outside of Atrium Health Connect.

Patients are to send completed Opt-Out Request Forms to Atrium Health CareConnect using one of the following methods:

Opting Back In:

If a patient has previously submitted a request to Opt-Out of the Atrium Health CareConnect and would now like to begin participating again or "Opt-In" to the Atrium Health CareConnect, they may complete a Request for Cancellation of Opt-Out Form.

By submitting a Request for Cancellation of Opt-Out Form, a patient's test results and medical information will be accessible to authorized health care providers through Atrium Health CareConnect. This includes any test or other medical information that was generated while the patient was opted out.
